What is your name : ¿Cómo te llamas? or if you don’t know the person that well, ¿Cómo le llama Usted?
Just to be clear, this actually means "How do you call yourself", but that’s how the Mexicans say it.
My name is : Me llamo Shaun (or whatever your name is)
Again, this means, "I call myself Shaun", but that’s how it’s said in Mexico.
Are you ready to go to lunch : ¿Estás listo para almorzar?
And dude, instead of asking, you could just go to Google, click Language Tools, and it’ll translate stuff for you. It’s not the most accurate thing, but it’ll get you through.
If not "Como se llama usted?", then maybe, "Que es su nombre, hombre?"
I’ll be ready for lunch in about an hour. You didn’t finish saying your name, though.

for a stranger "Como se llama?" or "Cual es su nombre?"
(not "como le llama" or "como se llamas" like some of the other answers said)

For a friend (female): "Estas lista para almorzar?"
For a friend (male): "Estas listo para almorzar?"
For a stranger (male/female): "Esta listo/lista para almorzar?"
